Passerby William Roberts, who witnessed the unusual scene, shared photos of the incident on his Instagram account.
He said he was on his way back home when he saw a human head slowly emerging from the drain hole, prompting him to pull out his camera and took some photos of her.
"I had just finished at the gym and was walking back home, camera in hand, doing a bit of a photowalk. Just at the corner of VA Rufino and Adelantado, I saw something that made me freeze — a head coming out of a street drain," Roberts said in his post.
"Instinct kicked in. I started snapping photos-couldn't believe my eyes. But then, she started running-towards me. For a second, I tensed, ready to defend myself if needed. The adrenaline hit hard. But just as quickly, a military/police guy on a scooter shouted "Wag!" so I stood down," he added.
Roberts' post quickly went viral on social media in just a couple of hours, catching the attention of authorities who promptly launched an investigation.
Upon inspecting the drain hole, law enforcers recovered various belongings of the woman inside the drainage system, including pieces of cloth and steel.
The Makati City Police identified the woman as a street dweller often seen roaming behind Makati Medical Center.
"The Makati Police Station immediately coordinated with Makati Commercial Estate Association and immediately checked that said canal and installed a metal cover to the said opening to avoid the same incident,” the police report stated.
The Makati Public Safety Department has also been directed to closely monitor the area.
