The film was a nominee for Best Picture in the Asian Next Wave Competition at the QCinema International Film Festival. Max Eigenmann continued to shine, winning Best Actress at the Entertainment Editors' Choice Awards (Eddys), sharing the honor with Janine Gutierrez. The film also received nominations for Best Screenplay and Best Editing.

At the prestigious Gawad Urian Awards, "12 Weeks" was nominated for Best Picture and Best Direction, highlighting the exemplary film that was produced at the taillend of the pandemic. Eigenmann won the Best Actress award for her outstanding performance, and Claudia Enriquez secured the Best Supporting Actress award, with Bing Pimentel also receiving a nomination in the same category.

The Young Critics Circle (YCC), Philippines, also acknowledged the film's excellence, awarding it the Best First Feature and Best Film. Eigenmann's performance earned her the Best Lead Performance award, underscoring her exceptional talent. Anna Isabelle Matutina received the Best Screenplay award and a nomination for Best Achievement in Film Editing.

The Society of Filipino Film Reviewers awarded "12 Weeks" with the Pinoy Rebyu Award for Best Screenplay and Best Film. Max Eigenmann won the Best Lead Performance award again, while Claudia Enriquez secured the Best Supporting Performance award. Bing Pimentel was again nominated for Best Supporting Performance, and the film was recognized with nominations for Best Director and Best Ensemble Performance. Additionally, it won the Best First Feature award.
