The Parañaque Health Plan Office (HPO) will be conducting free PhilHealth enrollment services for persons with disabilities (PWDs) for the whole month of June until July, announced the local government.
According to Mayor Eric Olivarez, the free PhilHealth enrollment is open to all PWDs who are not yet members of PhilHealth, including children with disabilities (CWDs).
Olivarez said PWDs who are not yet members of PhilHealth may visit their barangay to determine the schedule of enrollment.
Those who wish to enroll are required to bring the original and photocopy their updated PWD ID, government-issued ID that indicates their full name and birthday, a birth certificate issued by the Philippine Statistics Office (PSA) or from the Civil Registry Office with a registry number, and an original or photocopy of their Certificate of Barangay Indigency.
For CWD applicants, two sets of original and photocopy of any government-issued ID of the parents must be presented.
