The Pasig City Persons with Disabilities Affairs Office (PDAO) announced on Saturday, May 27, that slots have been opened for the free physical (PT) and occupational therapy (OT) program for persons with disabilities (PWDs) in Pasig City.
The application is on a first come, first serve basis. Only the first 100 applications for adults and first 100 for minors that will pass the office’s screening process will be accommodated.
Among the applicants who will be prioritized in the selection process by the PDAO are indigent PWDs, adults who have stroke or muscle dystrophy; as well as minors with developmental disorders. These disorders include Autism Spectrum Disorder (ASD), Attention Deficit Hyperactivity Disorder (ADHD), Spastic Cerebral Palsy, Down Syndrome, and Global Developmental Delay (GDD).
The qualified applicants will get vouchers that will be given to the specialists selected by the PDAO.
Applicants are required to submit an updated medical certificate, indicating the doctor’s recommendation for the patient to undergo therapy; a personal profile, specifying the type of therapy (PT or OT) with the full name, address, and contact number of the PWD; a photocopy of the PWD ID, a letter of request, certificate of indigency, and a whole body picture of the applicant.
Complete documents must be placed within a long brown envelope, labelled using the following format: PT / OT 2023; Type of Therapy; PWD ID number; PWD name; barangay; contact number.
Applications must be submitted to the PDAO at the city hall.
Deadline of submission is on Wednesday, May 31.
PDAO may announce the closure of applications, through their social media, even before the deadline if all available slots have been filled.
