The Valenzuela City government announced on Sunday, Feb.6, that it will open two sites for the COVID-19 vaccination of children ages 5-11 which will start on Monday, Feb. 7.
The vaccination sites are Pasolo Elementary School for the first district and Malinta Elementary School for the second district.
Parents or guardians can visit valtrace.appcase.net to list their children for vaccination. After the registration, a "VCVax" staff will send the vaccination appointment details through a text message or call.
The local government said the inoculations are strictly "by appointment" and only up to two parents or guardians may accompany their kids during the vaccination day.
The "ValTrace" quick response (QR) code of the parents/guardians, vaccination appointment letter or text message from VCVax, valid identification cards with photos (both the children and parents/guardians), proof of relationship, and medical certificate for children with comorbidities must also be presented.
According to the city government, parents who will accompany their children may present the original or certified true copy of their child's birth certificate, secondary identification card or an official document that shows the parents and recipient's name as their proof of relationship.
For guardians, they can present any of the following documents: special power of attorney; notarized authorization letter; written affidavit from parents/guardians and a valid ID; and certification from their barangay chairman that proves legal guardianship.
The city government also reminded those who will visit the sites to bring their own ballpens, alcohol, fans, drinking water, and to always observe minimum health and safety protocols.
