Malabon City launches free birth certificate registration program
The Malabon City government has launched the "Rehistro mo, sagot ko tungo sa pag-asenso" program that offers free birth certificate registration of children in the city for the month of February.
The local government said on Thursday, Feb. 3, that the program was launched in accordance with the celebration of the 32nd Civil Registry Month.
During the registration, the parents and their child must present a "Certificate of No Record" from the Philippine Statistics Agency (PSA).
They should also bring two documents of any of the following that show the full name of the child and the parents, as well as the date and place of birth of the child:
• Baptismal Certificate
• Immunization Card
• School Records- Form 137-E (Elementary), Form 137-A (High School)
• Certified photocopy of Voter's Affidavit
• Police, Court, or Nation Bureau of Investigation (NBI) clearance
• Social Security System E-1, E-4, or RS-1 forms
• Service record
• Marriage Certificate
• Certified photocopy of parent's marriage contract (if married)
• Barangay Certificate
The parents and children are required to present one valid ID each.
The IDs can be a barangay ID, a passport, a student ID, a police clearance, or tax identification number (TIN).
They must also present an affidavit of two people who know the child's birth details, an affidavit of consent to use the father's surname, and a certificate of indigency from the City Social Welfare and Development Department (CSWDD).
The forms for the affidavits are available at the city’s Civil Registry Office for free.
The local government said that residents can visit the City Registry Office located on the third floor of the city hall from Mondays to Fridays at 8 a.m. to 3 p.m. for questions about the program.
