Paranaque City LGU wants PhiHealth for students in preparation for F2F


Paranaque City Mayor Edwin Olivarez advised students aged 21 years-old to secure PhilHealth in preparation for face-to-face classes (F2F).

Olivarez said the Paranaque Health Plan Office located at the ground floor of the Paranaque City Hall came out with an advisory on April 4 on how to secure PhilHealth.

He said in securing PhilHealth, the students must visit first the Barangay Local Health Insurance Office and submit the following documents: photocopies of birth certificate issued by the Philippine Statistics Authority (PSA); latest school ID, latest school enrolment and registration card and barangay certificate of indigency.

The mayor said after accomplishing the said documents, the students can now visit the Paranaque Health Plan Office to assist them in filling up their PhilHealth member registration form (PMRF) and securing other document.

Olivarez said the office will also help students in securing at the City Social Welfare Development (CSWD) office the certificate of financial incapability and a copy of family profile.
