Makatizens' OsMak health records can be accessed online


The Makati City local government announced on Sunday, March 19, that Makatizens may now access their health records from the Ospital ng Makati (OsMak) via online as the city boosted its efforts to streamline its health services to its residents.

The LGU said that the initiative aims to provide fast and safe medical services to the residents and to ensure their adherence to the public health and safety protocols amid the Covid-19 pandemic.

According to the local government unit (LGU), residents can get their health records every Monday to Friday, 8:00 a.m. to 5:00 p.m.

The steps in requesting their health records are as follows:

Step 1: Scan your QR Code below, or open the browser and type the URL bit.ly/osmakrecords.

Step 2: Type your e-mail address and choose the language you will use. Be sure to type your correct e-mail address.

Step 3: Read the consent form and click Agree. By clicking Agree, you are authorizing the Health Records Personnel to access your personal information and hospital records.

Step 4: Fill up the information sheet by typing down your full name and date of birth.

Step 5: Fill up another entry about your Health Records Category whether you are In-Patient, E.R., Medico Legal, Outpatient, and dates of confinement or checkup and name of clinic or hospital.

Step 6: Choose the document requested.

Step 7: Fill up another document, correct and complete.

Step 8: Read the advisory.

Step 9: Upload the necessary files such as the SSS and Insurance forms.

Step 10: Wait for e-mail or text from Osmak Records Staff informing you when to claim the requested document.
