The Antipolo City government has started inoculating minors aged 12 to 17 on Wednesday, Nov. 3, in various COVID-19 vaccination centers in the city.
The names of the minors who have previously registered in Antipolo Bantay COVID portal, the time and date of vaccination and the venue of the vaccination site are posted on https://bit.ly/BakunadoKabataangAntipolenyo_November3_A_to_Z
The office of Mayor Andrea Bautista Ynares, in a post on Facebook, said those in the list and scheduled for Nov. 3 should have the following ready upon entering the vaccination site:
1. QR Code, PSA or LCR birth certificate or any alternative document that are stated in this link, -bit.ly/AlternativeDocuments ,
2. valid ID of the parent or guardian, valid ID of the minor or in the absence of the ID, or any valid ID stated in this link, bit.ly/List_Of_Valid IDs
3. If the minor is classified as Pediatric A3 or minors with comorbidities, the Standard Department of Health medical clearance, bit.ly/DOH_Medical_Clearance, the comorbidities are stated in this link, bit.ly/DOHPediatric_A3
The minors with confirmed schedule will receive a confirmation via text message from a Globe Number regarding the venue, site, and date of the vaccination of either Pfizer of Moderna vaccines.
Mayor Ynares reminded the parents and their children who are going to the vaccination site to follow the health protocols and be ready to sign the necessary forms to be given at the vaccination site.
The parent or guardian accompanying the minor who is scheduled for vaccination may also get vaccinated at the same time if he or she has not yet been vaccinated.
