Residents are advised to go to the nearest health center to register. They need to bring a valid ID, barangay indigency certificate, and PhilHealth ID. They need to answer a health assessment form which will be submitted to a health officer.

Those whose forms pass the assessment will be informed by the health office for the schedule to go to the MR Flores Health Center where they will be checked by doctors from VRP Medical Center.

If they pass the assessment by the health office and VRP surgeon, they need to get a clearance from the Municipal Social Welfare and Development Office and wait for the schedule of their operation at the VRP Medical Center.

A vehicle will be provided by the office of Mayor Miguel Ponce III for the patients.

Interested persons may contact the health office or Edgar Noel Castillo through Messenger or cell phone number at 09175253878.
