Comelec: COC forms available at website

Forms needed for the filing of certificates of candidacy (COCs) for the May 2022 elections can be downloaded from the official website of the Commission on Elections (Comelec).

The Comelec said that different forms needed for the filing of COC can be accessed at https://comelec.gov.ph/?r=2022NLE/COC/COCForms.

"All forms mentioned herein are available at the Comelec offices, free of charge, and may be photocopied, reproduced or downloaded from the Comelec official website," the poll body said in its Resolution No. 10717.

The forms available online include: List of Authorized Signatories (Annex A), Certificate of Nomination (Annex B), Affidavit of Sec. Gen. and President or Chairperson on the List of Substitute Nominees (Annex C), Certificate of Acceptance of Nomination (Annex D), Sworn Authority to Submit Certificate of Nomination and Certificate of Acceptance of Nomination (Annex E), Certificate of Nomination and Acceptance (Annex F), Statement of Cancellation (Annex G), and Authority To Submit Statement of Cancellation (Annex G-1).

Also available online are forms for Certificate of Candidacy for President (Annex H), Certificate of Candidacy for Vice President (Annex H-1), Certificate of Candidacy for Senator (Annex H-2), Certificate of Candidacy for Member, House of Representatives (Annex H-3), Certificate of Candidacy for Provincial Governor (Annex H-4 ), Certificate of Candidacy for Provincial Vice-Governor (Annex H-5), Certificate of Candidacy for Member, Sangguniang Panlalawigan (Annex H-6), Certificate of Candidacy for Mayor (Annex H-7), Certificate of Candidacy for Vice Mayor (Annex H-8), Certificate of Candidacy for Member, Sangguniang Panlungsod (Annex H-9), and Certificate of Candidacy for Member, Sangguniang Bayan (Annex H-10).

Forms also available at the website are for Authority to File Certificate of Candidacy (Annex I), List of Aspirants in the Queuing Area at 4:45 PM of Oct. 8, 2021 (Annex J), List of Aspirants in the Queuing Area at 5:00 PM of Oct. 8, 2021 (Annex J-1), Report on Non Receipt of Certificate of Candidacy (Annex K), Record on the Filing of Certificate of Candidacy (Annex L), Record of Certificate of Candidacies Not Received (Annex L-1), Daily Report on the Number of Aspirants who Filed their Certificate of Candidacy per Elective Position by Sex for the May 9, 2022 National and Local Elections (Annex M), Statement of Withdrawal (Annex N), and Health Declaration Form (Annex O).

The filing of COCs is set on Friday, Oct. 1 to Friday, Oct. 8.

The Comelec said that no person can run for public office "unless such aspirant accomplishes and files a sworn COC in the form prescribed by the Commission for the position aspired for."
