Here is the list of online registration forms for COVID-19 vaccinations in Metro Manila


All Metro Manila local government units are working round the clock this week in making sure that their constituents have a quick and fast vaccination experience so they can go home right away.

Here's the list of cities and their vaccination registration links:

Caloocan
Registration is via a google docs link, but the page now says that it isn't accepting registrants any longer.

Las Pinas
Fill up the form in this link, and make sure you have any government-issued IDs on hand.

Makati
It has it's own webpage and starts off by asking you to choose which category you are in. Take note that there are four different types of senior citizens for this city. Proceed to this link.

Malabon
All you need to do is answer one long page of questions through this link.

Mandaluyong
Having its own dedicated webpage for this, expect a lengthy one-pager to fill out and you're done. Click on this link.

Manila
Register here to get a vaccination slot. Though their site is down today, you may go their Manila Public Information Office Facebook page for urgent matters.

Muntinlupa
This site has one of the most extensive forms among the cities, but it's fairly easy to answer. Go to this link to start filling our the necessary details.

Navotas
This link will lead you to a quick one-pager form.

Paranaque
You need to sign up before filling out the form. Click on this link to proceed.

Pasig
The city will be using the Pasig Pass database for the COVID-19 vaccination roll out. If you haven't registered, go to this link.

San Juan
Prepare any government-issued ID and 1x1 photo for upload when you fill out this form.

Taguig
The city is using the Trace app for the COVID-19 vaccination registration, among others. Click on this link if you haven't signed up.

Quezon City
This city's residents should sign up in their E-services portal and apply for Quezon City ID before registering for the COVID-19 vaccine.

Valenzuela
Their contact tracing app is also being used for the vaccine registration. You will need to get a ValTrace QR code to be qualified.
