Cebu City Councilor Joel Garganera expressed disappointment after discovering that some contact tracing forms were not properly filled out.

Several commercial establishments here are requiring customers or visitors to fill out contact tracing form before they will be allowed to get in.

Garganera, who is the deputy chief implementer of the Inter-Agency Task Force Emergency Operations Center, was disappointed that there were individuals who made fun at this measure.

In his Facebook page, Garganera, posted a photo of a contact tracing form from a commercial establishment where a costumer listed his name as “optimus prime” and “planet cybertron” as his address.

There were also visitors in the same establishment that listed their names as “Doc Strange” and “Bruce Wayne.”

“Some people found this funny, but how many lives will be affected by making lies? This doesn’t make you superheroes. This makes you villains. Next time, an arrest should be made,” Garganera said.

“Do not follow this practice. This is not something to laugh about. Please fill-up your forms properly,” Garganera added.

Garganera gave some reminders when going to commercial establishments.

The reminders included bringing own pen and giving full and correct information.

Being specific with the address such as providing the sitio, barangay and the city is a must.

Penmanship must be eligible and one must sanitize after filling out the form.
