Taytay LGU encourages residents to register for COVID-19 vaccination


The local government of Taytay, Rizal is enjoining its residents to enlist themselves either manually or online for the planned vaccination program against the coronavirus disease.

Mayor George Ricardo Gacula, II said the local government is offering two options for the residents - manual or online registration using the TaytayVax Pre-screening Application Form.

The Taytay Municipal Health Office, in a post on its Facebook page, said those who may opt to manually register especially those who have no access to a computer or cellphone with internet connection, are encouraged to go to Taytay Emergency Hospital or at the 39 Health Centers in the town, to get the screening form.

The registrants are enjoined to fill-up the needed details otherwise, indicate “N/A” or “Not Applicable”.

Manual registrants should attach to the screening form, a 1x1 picture and a photocopy of one valid ID (either GSIS, SSS, OSCA, BIR TIN, UMID, voter’s or barangay ID, passport, driver’s license, or Philhealth).

Those who have submitted the manual application form need not fill-up the Online application form.
If they choose to pre-register online, the individual can go to the link : http://bit.ly/besttaytayvaccineform through a web browser, or visit the official website of the Taytay Municipal Government, munisipyo: https://www.taytayrizal.gov.ph/.../covid-19-vaccination, using their Google email account and fill-up the online form.
Online registration also requires the uploading of a valid ID (GSIS, SSS, OSCA, BIR TIN, UMID, voter’s or barangay ID, passport, driver’s license, or Philhealth).

As of February 25 , the town has recorded 43 active cases bringing to 2,372 the total number of COVID-19 cases since the pandemic started. There have been 2,238 recoveries and 91 have died.
